Anybody ever had a proper fitting boot and still get bruised toenails?

I'm finally getting new boots, the Scarpa Zodiak GTX. I've had these Vasque boots for maybe a decade. In the past 3 or so years, I've noticed that my toenails are getting bruised easily from longer days, and I don't recall that happening before. I've learned the ankle-lock lacing technique and I sometimes have to re-tie on the way down, but I'm able to keep the bruising at least to a minimum, often non-existant. The strange part: I feel like they're being bruised by the top of the toe box, not by smashing into the front so much. I'm trying to size my new boots properly, but I'm not sure my old Vasques were even sized incorrectly. I have a thumb's width of space between the end of my toes and the end of the insole. My questions:

  1. Is there a fitting fallacy I'm missing that I should pay attention to when getting my new boots?
  2. Has anyone else experienced bruising in part from the top of the toe box?
  3. Can bruising set in late in a boot's lifespan, even if it fits well?
